MELASMA VS PIGMENTATION: HOW TO TELL THE DIFFERENCE

One of the most common reasons patients book a consultation at my clinic is because they have been told they have “pigmentation” – yet treatments haven’t worked, or the problem keeps coming back. In many cases, the issue isn’t the treatment itself, but the diagnosis.


Melasma and pigmentation are often grouped together, but they are not the same condition, and treating them in the same way can lead to disappointing or short-lived results.


At Moreton Aesthetics, my approach is always to identify the type of pigmentation first, before recommending any treatment.

What Is Pigmentation?


Pigmentation is a broad term used to describe areas of excess melanin in the skin. It is most commonly caused by:

  • Sun exposure

  • Inflammation, such as acne or eczema

  • Age-related skin changes


This type of pigmentation often appears as freckles, sun spots or post-inflammatory marks. It is usually well-defined, darker than the surrounding skin and more responsive to treatment when managed correctly.


What Is Melasma?


Melasma is a chronic pigmentary condition driven largely by hormones and heat, not just sun exposure. It commonly presents as symmetrical patches on the cheeks, forehead, upper lip or jawline.


Unlike general pigmentation, melasma is:

  • Often deeper within the skin

  • Triggered by hormonal changes such as pregnancy, contraception or perimenopause

  • Highly sensitive to heat and UV exposure


This is why melasma frequently returns if it is treated aggressively or without long-term maintenance.


Why the Difference Matters


Treating melasma as standard pigmentation can worsen the condition. Overuse of heat-based devices, strong peels or excessive exfoliation may stimulate melanocytes further, leading to rebound pigmentation.


Pigmentation caused by sun damage or inflammation, on the other hand, often responds very well to targeted treatments when the skin barrier is respected.

Correct diagnosis is essential for safe, effective and sustainable results.


How I Assess Pigmentation in Clinic


Every pigmentation consultation begins with a detailed assessment. I look at:

  • Distribution and symmetry

  • Depth of pigment

  • Hormonal history

  • Previous treatments and responses


This allows me to determine whether we are treating melasma, superficial pigmentation, or a combination of both – which is very common.


Treating Pigmentation Safely


For non-melasma pigmentation, treatment plans often include:


Moxi® laser is an effective option for treating superficial pigmentation and early sun damage. It works by delivering controlled, non-ablative laser energy to stimulate cell turnover and collagen production, helping to break up pigment while improving overall skin tone and texture. Moxi® is particularly suited to patients looking for gradual, natural-looking improvement with minimal downtime, and can be safely used on all skin types, even when there is a light tan.


Consistency and sun protection are key to maintaining results.


Treating Melasma: A More Cautious Approach


Melasma requires a slower, more controlled strategy. My focus is always on calming the skin and reducing triggers before introducing corrective treatments.


This often includes:

  • Medical-grade pigment regulators such as Obagi Nu-Derm, prescribed carefully

  • High-performance antioxidants, such as SkinCeuticals formulations, to protect against environmental triggers

  • Strict daily sun protection and heat avoidance


When treating melasma, laser choice is critical. While some lasers can aggravate melasma if used incorrectly, Moxi® may be considered in selected patients as part of a carefully planned, combination approach. Its gentle, fractional technology allows us to target skin quality and superficial pigment without excessive heat, but it is never used alone for melasma. At my clinic, Moxi® is only considered alongside medical-grade skincare, pigment suppressors and strict sun protection.


In many cases, improvement comes from long-term management, not quick fixes.


The Role of Medical-Grade Skincare


Medical-grade skincare is the foundation of all pigmentation treatment. Systems such as Obagi Nu-Derm work by normalising cell turnover and reducing excess melanin production at a cellular level.
