TEAR TROUGH FILLER: MY APPROACH TO TREATING TIRED UNDER-EYES NATURALLY

Who it helps, when I recommend it, and when I don’t.

The under-eye area is one of the first places people notice ageing or tiredness. Patients often tell me, “I look exhausted even when I feel fine,” or “Concealer just isn’t working anymore.” In many cases, the issue isn’t skin quality alone — it’s structural change.

REFRESH YOUR EYES.

Tear trough dermal filler can be a very effective treatment when used appropriately. But it’s also an area that requires caution, precision, and honest patient selection.

At Moreton Aesthetics, I take a conservative, anatomy-led approach to under-eye rejuvenation because natural results are always the priority.

What Is the Tear Trough?

The tear trough is the groove that runs from the inner corner of the eye down toward the cheek. In youth, this transition is smooth.

With time, changes occur beneath the skin:

  • Fat pads shift

  • Bone support reduces

  • Ligaments become more visible

  • Skin thins

  • Volume is lost in the midface

This creates shadowing, hollowing, and the appearance of dark circles.

Importantly, many “dark circles” are actually shadows caused by structure, not pigmentation.

Why the Under-Eye Area Ages So Noticeably

The skin under the eyes is extremely thin, and the anatomy is complex. Small changes can create a big visual difference.

Patients may notice:

  • Hollows or indentation

  • Dark shadows

  • A tired appearance

  • Makeup creasing

  • A heavier transition between eye and cheek

Because this area sits in the centre of the face, even subtle ageing can affect overall facial perception.

How Tear Trough Filler Works

Dermal filler placed strategically can restore lost support and smooth the transition between the lower eyelid and cheek.

When done correctly, this can:

  • Reduce hollowing

  • Soften shadows

  • Improve light reflection

  • Create a fresher appearance

  • Reduce the need for heavy concealer

The goal is not to “fill the under-eye,” but to restore structural continuity.

Often, treating the cheek at the same time improves results significantly because the two areas are anatomically connected.

My Philosophy: Less Is More

The tear trough is not an area for aggressive filling.

I prioritise:

Careful patient selectionSmall volumesPrecise placementGradual improvementFacial harmony

Sometimes the best treatment is not filler — and I always discuss alternatives when they are more appropriate.

Who Is a Good Candidate?

In my experience, tear trough filler works best for patients with:

  • True hollowing rather than puffiness

  • Good skin quality

  • Minimal under-eye bags

  • Mild to moderate volume loss

  • Healthy lymphatic drainage

  • Realistic expectations

Younger patients with genetic hollowing can also benefit.

When I May Advise Against Filler

There are situations where filler is not the best option.

These include:

  • Significant eye bags

  • Fluid retention

  • Very thin or crepey skin

  • Poor skin elasticity

  • Prominent festoons

  • Certain medical factors

In these cases, other treatments such as skin quality improvement, collagen stimulation, or energy-based devices may produce safer and more natural results.

Honest guidance is essential.

What Results Can You Expect?

When performed well, tear trough filler should look subtle.

Patients often say:

“I just look less tired.”“People say I look well.”“I don’t need as much concealer.”

The goal is refreshed — not obvious.

Does It Hurt?

Most patients tolerate treatment well. I use techniques to maximise comfort, and the procedure is usually quick.

Mild swelling or bruising can occur, but downtime is generally minimal.

How Long Does Tear Trough Filler Last?

Results typically last longer than many people expect because movement in this area is limited.

Many patients see improvement for 9–18 months, sometimes longer, depending on metabolism and product choice.

Safety Matters

The under-eye area contains delicate anatomy and important blood vessels. Treatment should only be performed by experienced medical professionals with advanced knowledge of facial anatomy.

This is not an area for discount or inexperienced providers.

My Consultation Process

During consultation at my clinic, I assess:

  • Facial structure

  • Skin quality

  • Cheek support

  • Eye bags vs hollows

  • Lifestyle factors

  • Expectations

I then create a personalised plan, which may or may not include filler.

The Takeaway

Tear trough dermal filler can be a powerful treatment for the right patient, creating a fresher and more rested appearance without surgery.

But success depends on:

  • Correct diagnosis

  • Careful technique

  • Conservative treatment

  • Holistic facial assessment

Natural results always come first.
